Hylafax rocks.  You have to locate clients for it on windows (there's a bunch
of them) that do what you want to do.  There is one that looks like a print
driver to windows and pops up a dialog when you print to the fax to put in
the phone numbers, etc.  Even has an addressbook that you can use.

Hylafax will receive faxes, too.  There are windows clients that will pull
the faxes from the hylafax server, too.  Check out hylafax's site.  All kinds
of resources
