I know you can edit and make forms in Adobe Acrobat 7 Pro. You could also
take a look at PDF Converter and PDF Create from Scansoft.

    Does anyone know how to make an existing pdf form fillable. Can it 
be converted to another format edited and then converted back? Or is 
there an application that can do the editing in the fields I need?
