But actually the form elements look ok..i finally got what you're trying
to do..(basically, your just doing the reverse of what I suggested)

I had these in functions, but exanded them out since I'm trying to debug
the problem.  ALso, the user/pass is no working yet, so just click
submit
button to view the results.
