I'm totally in love with postgrey. For me at least it kills a TON.
It'll kill you if you get hit with a backscatter tsunami, but it's been
worth it.
Postgrey+RBLs+a few other of the common main.cf recommendations have
made a hug difference of what actually ends up in people's inboxes.
